Firstly, thanks for making and publishing this, it is very useful. I will mention every-time I need to make test certs in a work env. I will try to get you sponsorship (you have saved time and money).

It would be a nice extension to be able to make a cert of XXX type, rather than "a cert". For example, I normally use EC (p256) certs.
Maybe add a --algorithm flag, but if absent it just runs your current implementation.
Assuming there is an underlying openSSL library (or similar), this should be an integration addon, rather than making cert algorithms yourself.

From the number of open pull requests, I cannot add this feature to the main repo quickly.
