Refs #386 (part 1 of 2: Home Assistant read-back. Skill validation follows in a second PR.)

Summary

After a Home Assistant action, Sentient now reads the device back and only calls it done when the device really changed. Before, only the call's own result was checked (#380), and Home Assistant says "done" even when a bulb ignores it.

home_assistant_turn_on / turn_off / set_temperature / open_cover / close_cover / lock / unlock / set_alarm
  service call -> result check (as before)
  Tool.verify = _reads_back(action)            # new
    wait 0.5 s, then GET /api/states/<id> about once a second
    for integrations.home_assistant_check_s (default 4 s, 0 = off, max 10)
    ok      -> state is as asked (on/off, locked/unlocked, open/closed, armed_<mode>/disarmed, hvac_mode, temperature)
    moving -> still opening/closing/locking/unlocking/arming/pending when time is up: fine, detail says so
    wrong   -> old state or unavailable: {ok: false, detail: "Hallway light still reports off", missed: [entity ids]}
  scenes, scripts, kinds with no plain on/off -> no read-back (as today)

Where the failure shows up:

 Agent._record (chat, task runs, subagents)
   check = outcomes.check(...)
+ if the read-back failed: the tool message the model reads ends with
+ "[Sentient checked afterwards and it did not work: <detail>. Tell the user plainly; don't say it worked.]"
 Agent._quick_turn (quick light commands, no model)
- reply "Turned on the hallway light." # never checked
+ check each call; tool_result.check, done.checks, saved message checks
+ read-back failed -> reply "I tried, but Hallway light still reports off."
 outcomes._targets
+ "home" checks target the call's entity ids, so another light coming on later doesn't settle the failure

Task runs need nothing new: a failed read-back already ends the run completed_with_errors (Needs attention) through #380.

Docs: docs/API.md section 4 "Checking the outcome" and section 23 quick commands. CHANGELOG. New config key integrations.home_assistant_check_s, shown in Settings.

Evidence

Tests (tests/integrations/test_home_assistant.py, FakeHA gained devices that ignore calls or report late):

  • an obeying lamp passes, an ignoring strip fails with "Bedroom strip still reports off" after reading again;
  • a lock that reports locked only on the third read passes;
  • what counts as done per kind (media player "not off", jammed lock, unavailable, alarm modes, thermostat mode and temperature);
  • a garage door still opening is fine with a note; a scene isn't read back; 0 turns it off;
  • through the agent loop, the model's next call sees the "checked afterwards" line and done.checks has the message;
  • a quick command says "I tried, but Bedroom lamp still reports off." and, once the lamp obeys, "Turned on the bedroom lamp." with a read_back check;
  • another light coming on doesn't settle the strip's failure; the strip coming on does.

Full suite: 2304 passed, 2 skipped. Ruff clean. No desktop changes (the check display is from #380).

Real end-to-end test. Engine from this worktree on port 8787, fresh SENTIENT_HOME, random gateway token. The #372 fake Home Assistant (Maya Rao's home) on 127.0.0.1:18386, with a new Hallway light started with --ignore light.hallway: it accepts every call and does nothing. The other devices obey.

Before this change, the same call returned {"done": true} with no read-back and the reply was "Done".

  1. claude-code/sonnet chat, "Can you put on the hallway light and the bedroom lamp for me?":
    tool_call home_assistant_turn_on {"entity_id": "light.hallway,light.bedroom_lamp"}   (approval: allow)
    fake-ha: IGNORED by light.hallway (state stays off)
    tool_result {"done": true, ..., "now": [{"name": "Bedroom lamp", "state": "on"}]}
      check {"ok": false, "detail": "Hallway light still reports off", "how": "read_back"}
    reply: Bedroom lamp is on, but the hallway light didn't actually turn on, it still shows off. Want me to try again...
    done.checks[0].message: "Sentient tried to run the home action, but Hallway light still reports off."

  2. claude-code/sonnet chat, door unlocked from outside, then "Lock the front door and set the hallway thermostat to 22":
    home_assistant_lock check ok: true, how: read_back, set_temperature check ok: true, how: read_back. Reply: "Front door's locked and the hallway thermostat is set to 22°."
  3. Quick commands (no model):
    "turn on the hallway light" -> check ok:false, missed ["light.hallway"] (4.3 s, the full wait)
                                    reply: I tried, but Hallway light still reports off.
    "turn off the bedroom lamp" -> check ok:true, read_back (565 ms)
                                    reply: Turned off the bedroom lamp.

  4. qwen3:8b chat (ollama_chat/qwen3:8b, Ollama lock held), "could you get the hallway light on for me?": find_entities, then turn_on (allow). Check ok: false. Reply: "The hallway light didn't turn on. Let me check if there's a switch or scene controlling it instead..."
  5. qwen3:8b task "Turn on the hallway light and the bedroom lamp", planned and run by qwen3:8b:
    • Run status: completed_with_errors.
    • run.error: "Sentient tried to run the home action, but Hallway light still reports off."
    • Checks: hallway ok: false, missed light.hallway; bedroom lamp ok: true, read_back.
    • The executor saw the failure, re-read the hallway light with get_state, and said "The hallway light is still off."
    • Small-model quirk, unrelated to this change: qwen planned the task as triggered on a state change, so it was started with run-now.

Merge Danger

Door: two-way

Blast Radius: small

Home Assistant actions now wait up to 4 s when a device doesn't change. A device that obeys answers on the first read, 0.5 s after the call, so a quick light command takes about 0.5 s instead of about 0.1 s. Set home_assistant_check_s to 0 to go back to the old behaviour. The model note and the quick-command check apply only when a read-back fails, and today only Home Assistant, file_write, Gmail and Calendar have read-backs.

Decision for the owner

  • Settle wait. The read-back waits 0.5 s before the first read, as the issue asks, so a device that briefly shows the new state and then falls back is still caught. This costs quick light commands about 0.5 s. Reading at once and retrying only on a mismatch would keep them near 100 ms, but could pass a device that reverts.
  • Still moving counts as fine. A garage door or lock still opening or locking when the time runs out counts as fine, with a note, not as a failure.

Summary by CodeRabbit

  • New Features
    • Home Assistant actions now check whether supported devices reach their expected state before reporting success. If a change isn’t confirmed or the state can’t be read, chat, quick commands, and task runs report the outcome.
    • Configure the verification wait in Settings. It defaults to 4 seconds and can be set to zero to disable verification.
    • Devices still transitioning when checked may be reported as successful, with details.

Merge Risk: 🔵 Low · up to ac529

Some thermostat commands can be reported as successful without confirming the requested temperature. Correct the read-back check before merging, or accept this bounded risk.

Security architecture risk: 🔵 Low · up to ac529

The change improves detection of unsuccessful device commands without expanding device permissions. The main introduced risk is interruption after a command has executed but before its result is saved. In addition, unavailable verification and devices still moving can continue to produce success responses, so read-back should not be treated as proof that a security-sensitive action finished.

Retained concerns

  • Medium · reliability · inferred: The new polling phase separates an executed device action from its persisted result. Cancellation during verification propagates before the current check, tool message, and result event are recorded. This widens the interruption window compared with the base, including for approved lock and alarm actions, leaving the conversation or checkpoint without a recorded outcome for an action that may already have changed physical security state. The inspected stopped-turn handler preserves accumulated checks, not the in-flight check; independent audit or recovery guarantees were not established.
Security review details

Security Blast Radius

  • inferred — The assessed exposure is the connected Home Assistant installation and the entities permitted for each command, potentially multiple entities in one call. Incorrect outcome reporting can affect physical-security devices as well as ordinary appliances, but the new verifier does not itself expand the acted-on entity set or credential authority.

Trust Boundaries and Controls

  • observed — Read-back failure details include Home Assistant names and states and are appended to model-facing tool content. Those values were already exposed by entity and service results, and write results are not marked as outside content by the default classification. The inspected comparison therefore does not establish a newly introduced Home Assistant content-boundary bypass or additional tool authority.

Resilience and Maintainability Implications

  • observed — Read-back is not an authoritative security-completion guarantee. Exceptions retain result-only success, while locking, unlocking, arming, and pending states can return ok=true with detail. Only failed read-back checks generate the model warning. Result-only success predates this PR; these paths limit the new verification guarantee rather than demonstrate increased authorization exposure.

Hardening Proposals

  • proposed — Consider distinct confirmed, pending, and unverified outcomes for security-sensitive devices, with user-visible pending or unavailable-verification status. Require confirmed read-back before treating a previous read-back failure as recovered.

🟡 Minor · Filter set_temperature outcome targets to the serviced domain. · outcomes.py:224-227

sentient/agent/outcomes.py:224-227
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Filter set_temperature outcome targets to the serviced domain.

When home_assistant_set_temperature receives mixed climate and water_heater IDs, it services and verifies only the first domain. The outcome check still records every original ID in targets. A task can therefore complete while claiming success for an unserviced entity, and later checks can incorrectly treat that entity as covered.

Apply the same first-domain filter only when constructing set_temperature outcome targets. Keep the existing read-back filter unchanged.

🟡 Minor · Treat a missing or non-numeric reported temperature as unconfirmed. · home_assistant.py:477-493

sentient/integrations/plugins/home_assistant.py:477-493
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Treat a missing or non-numeric reported temperature as unconfirmed.

When want contains a temperature, _compare only rejects a numeric value that differs from the requested value. A stable Home Assistant state with no numeric attributes.temperature therefore returns "ok", so the read-back can report success without verifying the requested temperature. Keep the existing moving-state behavior, but return "wrong" when a non-moving state does not report a numeric temperature.

Suggested fix
     if "temperature" in want:
         set_to = (s.get("attributes") or {}).get("temperature")
