[Feat]: Add a migration issue template #1530

Description
As this repo is planning a migration, a ISSUE_TEMPLATE dedicated to it must be created, so the contirbutors know what fields should be put in the issue and what changes they are going to make are understood by the maintainer.

I'll create a dedicated migration issue template that captures all the necessary fields (scope, what's being migrated, dependencies, breaking changes, etc.) to help contributors and maintainers track migration progress.
