Changelog not tagged as v0.5.0, and also its a lot more verbose then that of legion, even though the changes there were more extensive

Fixes from a pre-release review: escape raw HTML in rendered markdown (XSS via LLM responses), honor the
:transportrouter option, survive remote pids and deleted agent modules, evict oldest events past the cap, monitor HumanTool askers, guard telemetry sends during tracker restarts, exclude local debris from the Hex package, plus smaller doc/CSS cleanups. The CHANGELOG has the full list.
